For all group acts (non-solo) please list each and all members here. The group will not be able to audition unless an individual form is also submitted for each member of the group:

ATTENTION PARENTS: ONLY SUBMIT THIS FORM IF YOU ARE POSITIVE YOUR CHILD WILL AUDITION ON March 13th.

Acts will be selected on a first-come first-serve basis. Acts selected to audition will receive a parent permission slip that will be required to be completed and signed.

As the parent/guardian of the student named above, please acknowledge the following::
1. The information provided above is complete and accurate
2. This form is being completed and submitted with my knowledge
3. I will make every effort to see that my child attends his/her designated audition on March 13th, 2012
